Adult Placement Scheme

All Adult Placement Scheme’s within Cambridgeshire are registered with CQC. The Schemes support Carers who offer long term care, short breaks, including day care, for adults with a learning disability. The Care is delivered in the Carers own home or the Service Users home, and out in the community.

The Outreach Scheme offers Service Users the opportunity to develop basic living skills, encourage a healthy eating programme and to enjoy new experiences, new friendships and to gain independence.

Adult Placement Carers are recruited and supported by the Scheme, which has to be registered with CQC in accordance with the Care Standards Act 2000 and has to comply with the Adult Placement Schemes (England) Regulations 2004.

The Schemes promote positive relationships between Service Users and Carers by enabling people with a learning disability to experience life in the community and to share the family life of the Adult Placement Carer.

The support offered is developed around the needs of the individual and careful “matching” takes place to make sure that both Carer and Service Users are right for each other.

Service Users know that their needs have been fully assessed and they are involved whenever possible in their Service User Plan. The aim is to equip people with the knowledge, skills and confidence to maximise their individual potential.

Regular Carer / Service User reviews and monitoring of the placement are included in the care plan. Service Users send the Scheme their survey forms in order to promote new ideas and choices from individuals.
