About this organisation
The Transitions Team works with young people with disabilities age 14+, and their families as they plan for adult life.
The Team aims, through practical advice and guidance, to ensure that young people make informed choices and develop the right skills for independent, fulfilled and self directed futures.
The Team is made up of 6 experienced social workers/care managers who work across Cambridgeshire, helping young people develop support plans which detail their needs and how they could be met.
The Team strongly believes that young people with disabilities should have the same opportunities as their non-disabled peers.
